Welch, 82, who died on Feb. 15, rose to prominence as a sex symbol with her roles in “One Million Years B.C.,” “Fantastic Voyage” “Myra Breckinridge” and “Kansas City Bomber.”

According to Welch’s death certificate in Los Angeles County, obtained by Entertainment Tonight and TMZ, Welch’s cause of death was listed as cardiac arrest. The document also lists Alzheimer’s disease as an underlying cause of the actress’ death.

TMZ was the first outlet to report about Welch’s death certificate.

Welch had not publicly spoken about having Alzheimer’s disease, according to the Los Angeles Times.

She won a Golden Globe for her performance as Constance Bonacieux in “The Three Musketeers” (1973), according to Entertainment Tonight. She received a Golden Globe nomination in 1987 for her role in “Right to Die,” the entertainment news website reported.

Welche was honored with a star on the Hollywood Walk of Fame in 1994.

Welch always downplayed her reputation as a “bombshell” actress, the Times reported. She told the newspaper in 2010 that she was determined to prove that she was more than just a sex symbol.

“As life goes on, you get more valuable as a person. Many women look better,” she said. “Personally, I think I look better because I have lived and I have a different kind of aura about me having lived.”
