Local contractor facing $4.7M in fines after deadly Cape Cod trench collapse

A Franklin-based construction company is facing millions of dollars in fines following a deadly trench collapse that killed a worker on Cape Cod last year.

Miguel Reis died in November after a trench he was working in collapsed at a construction site on South Shore Drive in Yarmouth, burying him in sand.

Federal investigators say the incident could have been prevented. The Occupational Safety and Health Administration (OSHA) has issued seven violations against Revoli Construction in connection with the collapse. The proposed penalties total nearly $4.7 million.

The company now has two weeks either to comply with the findings, request an informal conference with OSHA, or formally contest the citations.

The collapse happened on November 18, just before 9 a.m., in front of the Skipper Restaurant and Chowder House. Emergency crews responding to the scene found multiple workers trapped.

Two other workers were pulled from the trench and taken to the hospital with injuries.

The incident remains under investigation.
