Man dies after falling down elevator shaft at construction site

HAVERHILL, Mass. — A 41-year-old man died after he fell down an elevator shaft in Haverhill, the Essex DA's office said.

Emergency crews were called to a construction site at 98 Essex Street at around 10:20 a.m. on Tuesday for a reported accident.

At the scene, they found the 41-year-old, identified as Carlos Ortiz, who had appeared to have fallen down the elevator shaft. He was pronounced dead at the scene.

OSHA and Essex State Police Detective Unit along with Haverhill Police and State Police are actively investigating this incident.
